** IMPORTANT NOTICE ** LATEST ADOBE READER CREATES PROBLEMS WITH FORMLINK FORMS

To Valued Clients:

The latest version of Adobe Reader (version 9.3X or higher) creates some compatibility issues with Formlink. Existing users of Formlink must NOT update to the latest versions of Adobe Reader. We recommend that users stay on version 8.x. We are working on possible workarounds for compatibility with these latest patches however we have no ETA at this time. This form is to be completed and signed by the manufacturer or exporter of the goods entering into the United States who has knowledge that the goods qualify for Nafta Used for goods valued at $2500US or less. Updated 4/7/2003.
U.S. Department of Transportation - VEHICLE DECLARATION
Declaration of Motor Vehicles and Motor Vehicle Equipment Subject to Federal Motor Vehicle Safety, Bumper and Theft Prevention Standards.
U.S. Certificate of Registration (CBP Form 4455 - Posted August 17, 2009)
This form is used to register goods before they leave the U.S., if intended on returning the goods after use in the foreign country.
U.S. Single Country Declaration
This form is used for textile products made of fabric/yarns from one country.
U.S. Multiple Country Declaration
This form is used for textile products made of fabric/yarns from more than one country.
Application for Exportation of Articles Under Special Bond (CBP Form 3495 - Posted June 1, 2009)
This form is used for the export of articles previously entered into the U.S.A. temporarily and without the payment of duty.
U.S. Radiation Declaration (FDA Form 2877 - Posted May 29, 2009)
This form is required for any product that emits radiation.
U.S. Toxic Substance Control Act Certification (TSCA)
This form is required for any product that contains toxic chemicals controlled under the Toxic Substance Control Act.
Notification of Temporary Importation Bond (TIB) Shipment
This form is to request a Temporary Import Bond (TIB) for items imported into the U.S. on a temporary basis.
Blanket Statement of Non-Reimbursement
 
FCC Form 740 (FCC Form 740 - Posted May 29, 2009)
This form is required for electronic goods being imported into the United States. If a product conforms to FCC, it will have a FCC number on the back of the product.

This form is required when importing peppers into the United States.
